Licence books will be sent out to the address given, for new members. Please make sure all details are filled in correctly together with a photograph upon return.

Your annual karate licence will be updated upon subscribing and automatically renewed each year.

Please remember to unsubscribe should you wish to cease training within the club to avoid further payments.

Commencement of activities such as courses, gradings etc. maybe carried out once this form is submitted and payment made.

Having an up to date licence is the responsibility of the student.
Unsubscribing is the students responsibility if no longer required   to avoid unwanted payments.
