Karate Licence

Licence books will be sent out to the address given, for new members.
Please make sure all details are filled in correctly together with a photograph upon return.

Your annual karate licence will be updated upon subscribing and automatically renewed each year.

Please remember to unsubscribe should you wish to cease training within the club to avoid further payments.

Commencement of activities such as courses, gradings etc. maybe carried out once this form is submitted and payment made.

Having a up to date licence is the responsibility of the student as is the unsubscribing if no longer required.   Please remember this to avoid unwanted payments.
